1) One Copy Passport Size Photo

2) Signature on White page

3) Discharge Book and Service Number

4) PAN card Photo Copy or Original

5) Aadhaar Card Photo Copy or Original

6) Mobile No (for OTP)

7) New PPO (SPARSH PPO)

8) Old ECHS Card (16 kb /32 kb)

9) Applicant's Bank passbook 

10)  Email ID (if available)

11) Blood Group

12) Name of Last Served Regiment / Place 

 

Documents of  Dependents1

1) One Copy Passport Size Photo

2) Signature on White page 

3) Blood Group

4) PAN card Photo Copy or Original 

5) Aadhaar Card Photo Copy or Original

 Note Please : 

🔹Son over 25 years can not be considered as a dependent of the Applicant.

🔹If Applicant's PPO have not migrated to SPARSH yet then bring previous PPO. And submit previous PPO instead of SPARSH PPO.

🔹Applicant having old 16 kb ECHS card will get Temporary Slip instant after application form submit. Applicant can use that Temporary slip for taking medical benefits until delivery of new card.

🔹Applicant having old 32 kb ECHS card will not get Temporary Slip instant. Applicant have to wait for few months to generate his/her Temporary slip.
