An ex-serviceman (retired defense personnel) can claim reimbursement for pharmacy bills or medical expenses incurred at local or outside pharmacies, provided the expenses with his own money. Additionally, if a patient is admitted to a hospital or diagnostic center that is not part of the ESIC Hospitals, they may also claim reimbursement for the medical bills.

🔴 Documents For Pharmacy Bill Claim

1) ECHS Card (64 KB) of Ex-Serviceman

2) ECHS Card (64 KB) of Dependents (if Medical bill issued for Dependent)

3) Mobile Number which is registered with ECHS Card (for OTP)

4) Bank Pass Book or Bank Cancel Cheque

5) Doctor's Prescription from Hospital

6) Bill of Pharmacy 


🔴 Documents For outside of hospital or Diagnosis center (Out Patient)

1) ECHS Card (64 KB) of Ex-Serviceman

2) ECHS Card (64 KB) of Dependents (if Medical bill issued for Dependent)

3) Mobile Number which is registered with ECHS Card (for OTP)

4) Bank Pass Book or Bank Cancel Cheque

5) Doctor's Prescription from Hospital 

6) Bill of  Hospital or Diagnosis center

7) Medical Report 

8) Sanction Letter 

 

⭕️ NOTE

🔹 Candidate have to submit his/her application form along with documents to the polyclinic within 7-10 days after online submission.

🔹 Documents to be submitted to policlinic below (Keep the original documents with you)

  • Online Application Print Out
  • Bank Passbook / Cancel Cheque Zerox copy
  • ECHS Card (64 KB) of Ex-Serviceman Zerox copy
  • ECHS Card (64 KB) of Dependents (if Medical bill issued for Dependent) Zerox copy
  • Medical Report (if Available) 
  • Bill of Pharmacy
  • Doctor's Prescription from Hospital
  • Addhaar Card of Applicant Zerox copy
  • Sanction Letter (for Out Patient)
