 🔴 Documents for Death Certificate

✅ Documents of Dead Person  : 

1) Aadhar Card Original copy of Dead person

2) Doctor's Certificate / Hospital's Death certificate

3) Burning Certificate (Ghat Certificate) from municipality or Panchayat

4) Govt Payment [GRIPS  Challan] (No need if death within 30 days)

6) Court Affidavit / Hospital Area Municipality Permission (No need if death within 30 days)

7) Father's / Mothers Name of Dead person

8) Form 2 (Authorized by health officer of death place along with stamp and signature) 

 

✅ Documents of Applicant (Relative of Dead Person) : 

1) Aadhar Card / Voter Card Original copy

2) Mobile (for OTP)

 ⭕️ FAQ : 

🔹 Govt will take Payment [GRIPS  Challan]. If Date of death 30 days to 1 year =  Govt will take fees Rs 2, If Date of death more than 1 year = Govt will take fees Rs 10. No fees required if death within 30 days. Click here to see Government Notification.

🔹You have to wait after application until Govt authority's approval.

🔹 Mobile, which is given at the time of online submission required for OTP to Download Death certificate from website.

🔹 Try to apply for death certificate online within 30 days of death.
